Outside Event Request

1. Request for Van Use: Use this form to ask for a van when you need to transport people. This could be for trips, events, or meetings. This will reserve the van when you need it, and it helps us keep track of van use.

2. Outside Event Request: Use this form to get permission for events outside our regular activities, like fairs, cookouts, and non-scheduled parades. Describe the event, its purpose, and the benefits. This ensures the event fits with our organization’s goals and rules.

3. Fundraising Activity Form: Use this form to propose a fundraising event or raffle. Explain the activity, date, and location. This helps us oversee fundraising efforts and keep things in line with our mission.

   • ​​If you wish to host a fundraiser for Shriners Children’s, it will have to be approved by the hospital, and there is a separate form to fill out.
